Sylester Sanchez, not his real name but the name I new him by, a journeyman ironworker was descending a ladder at NCCP unit no.2

Nebraska City Nebraska the ladder descended approximatly 20 ft. to a platform with lifeline access to a safe area, the ladder was also equipped with a yo-yo a deceleration device that eliminates the chance of a fatal fall. Mr Sanchez, decided that the yo-yo was unnessicary and proceded to descend the ladder. he also passed the lifeline without attaching his harness laynard to it. What Mr. Sanchez failed to notice was the fact that the landing had been removed to facilitate the placing of a piece of pipe and the 200 ft fall,

bouncing off the iron all the way down to the dirt thus his entry into darwin history

Safety measures such as harnesses, yoyos, &c are generally brought on as a result of tragic accidents with the intention of preventing them in the future (often, expensive lawsuits are also part of the impetus). These safety measures cost money but have saved countless lives. Nevertheless, there's always one or two bozos who just don't get it. As Bruce said, this is only a PA unless you can obtain a copy of the OSHA report (in which case I will gladly elevate it to full DA status). Thanks!
